4. Provide us the web address under which the link has been published.
5. Provide your contact details which must include your full name, address, and contact number.

You should also know that, under the DMCA, claimants who make misrepresentations concerning copyright infringement may be liable for all damages incurred as a result of the removal of the material, attorneys fees, and court expenses.
A proper notification must have the information mentioned above or may be ignored.
